We would like to ask for your support in completing this year’s NYC School Survey. The 2020-2021 survey is open for all of our families to complete. We are currently at an 11% completion rate.  

The NYC School Survey is available online ( will log in using an “f” before their child’s OSIS#.

 Families should provide feedback for each child .

Surveys are available and translated into all 9 covered languages.  If you do not have access to the internet to complete the survey online, please call the school at 718-423-8138 or e-mail Ms. Cho ( or Ms. Lam ( and we can mail or send you an e-mail attachment of the school survey for you to complete.
